Forms, Documents and Links

1.  Apprenticeship Application - This form is to be completed by new apprentices entering into the program.  The employer will add the appropriate information and then submit to the SFMAP Board for review and approval.

2.  Apprenticeship Agreement  - This form is for new manufacturing companies who wish to join SFMAP.  Along with the completion of this form, there is a one-time application fee of $100.00 (check payable to SFMA).  The form is completed by the manufacturer and approved by SFMAP.

3.  Wage Survey - This form is used to verify annual wages and demographics, and must be completed annually by SFMAP participating companies.

4.  Florida Department of Education Apprenticeship  - this link will take you directly to the Apprenticeship   page on the FLDOE website.

5.  Donation Request Form - This form is for any party who would like to donate money to SFMAP.  Please note that there are various levels of support which can be provided.  Please see form for details.

6.  Monthly Timesheet - This form is to document class attendance.
